What Needs to be Done to be Eligible to Practice:

• Signed “Activity Code of Conduct” Form
• Signatures indicating parental and student acknowledgement of the “Activity Code of Conduct”.
• Medical Evaluation Report
  • Signature by a medical authority provides clearance for athletic participation every year.  Requirements include a minimum of a complete physical every (2) years, with a Health Care Provider clearance (signed off) the alternate year.
• Emergency Medical Treatment Card filled out.  (This form is included within the “Activity Code of Conduct” packet)
• The name of the Insurance carrier and policy number MUST be listed on the insurance form.
• Signed “Concussion Information Sheet” by parent(s) and student-athlete
• ASB Card purchased ($45) and all outstanding fines MUST be cleared prior to eligibility.
• Participation Fee Paid ($120).
• Maintain all academic and attendance eligibility requirements.

 All the above items must be completed and turned in to the Athletic office by Thursday,
